How it Works

Initial Assessment & Action Plan  

With a complimentary phone consultation to get a better understanding of an individual’s unique situation, a follow up appointment is arranged where the care manager travels to meet with the prospective client and family member(s) if they are available to be present. 

The care manager will want to gather as much information as she can during this initial visit in order to develop a comprehensive care plan that will include recommendations, referrals for local care options and referrals to resources.

The care manager will then find out what you can do yourself, what can be done by other family members, and the extent of care management desired, and matches this to the priority lists and economic abilities, and then helps to arrange for and monitor services.
