Did you know that Geriatric Care Manager visits can increase the vigilance of care your loved one receives? When a facility knows that we make cold calls to monitor the care of someone, it can make a difference in the quality of care your loved ones receive. We check the care charts, visit with the client to check skin integrity, weight loss/gain, hygiene, nutrition, and psychosocial health and provide an overall evaluation.
If you have a family member in a nursing home or other facility in the San Francisco Bay Area, you may want another pair of eyes and ears to check on that person regularly. With so many baby boomer adult children living across the country and world from where their parents still live, an objective evaluation about how a loved one is doing cannot be emphasized enough.
The Monthly Monitor Service includes:
One monthly visit, 1-hour
4 -6 emails
3-5 telephone calls
Travel to and from facility
(surcharge could apply if facility is greater than 10 miles away)
Written reports to family member(s) with Care Manager's evaluation and suggested follow up tasks based on visit outcome.
What it costs: $250 per month
